PROCTOR HOCKEY WAIVER OF LIABILITY & OTHER INFORMATION
PLEASE READ THIS - THERE IS IMPORTANT INFORMATION YOU NEED TO KNOW
I hereby release, waive, discharge and covenant to hold harmless the Proctor Amateur Hockey Association, its Board of Directors, officers, volunteers, agents, contractors, and employees (hereinafter referred to as (“releasees”) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or relating to any loss, damage or injury, including death, that may be sustained by me, or to my dependent or dependents, and property belonging to me while participating in an event at the St. Luke's Sports & Event Center or as a member of the association’s regular functions.
I acknowledge that I am of at least 18 years of age and a competent adult. I am fully aware that participation in the sport of ice hockey can be dangerous, and that participants and spectators alike are at risk for injury, and in rare circumstances, even death.
I hereby acknowledge that while participating in an event of the association upon any of its premises, including but not limited to its outdoor skating facility, and parking lot, I am fully aware that flying pucks and/or other equipment may cause injury or death. In addition, I am aware that walking paths near or around the vicinity of the playing surface, especially the outdoor one, vary in temperature, and due to the refrigeration system or weather, they may become wet, slippery, or otherwise hazardous for foot travel without warning. While the association makes every attempt to maintain a safe environment for its members and guests, I hereby declare that I have been informed of and agree to incur any and all risks associated with participation in any event of the association.
I further hereby agree to indemnify and save and hold harmless the releasees and each of them, from any loss, liability, damage or costs they may incur due to my participation, or my dependent’s participation in any event of the association, whether caused by the negligence of any or all of the releasees, or otherwise. Further, I acknowledge that I have read or will read prior to my child or children’s participation in any association event the Proctor Amateur Hockey Association’s Guidebook for players and parents. This is available for reading online at the association's website, www.proctorhockey.com.
Further, I acknowledge that I have read the USA Hockey player’s code of conduct and understand that the consequences for not abiding by the guidelines and rules it sets forth will be implemented according to the steps outlined in the association’s guidebook.
Further, I agree that I have paid all registration and fundraising fee(s) in full or have a scheduled payment plan at the time of registration. I acknowledge that without having paid or scheduled the registration fee and fundraising fee, my child, children, or dependent(s) will be denied all participation in association events, to include practices, games, tryouts, open ice time, weight training, or training camp.
I also understand that if I am registering a returning player on August 10, 2023 or after, I will be subject to a $50 late fee that will be charged to my account by PAHA.
